Smoothened receptor in the Hedgehog pathway; activated when PTCH1 releases SMO repression after HH ligand binding → GLI transcription factors → BCL-2, cyclin D1, and MYC. Mutated in ~40% of sporadic BCC; vismodegib and sonidegib are approved SMO inhibitors for advanced BCC.
